CARE Program

Comprehensive Assistance with Recovery Empowerment (CARE) Program:

This adult (ages 18 and over) program provides person-centered, individually planned treatment, rehabilitation, and support services to identified clients with serious mental illness (SMI) including SMI Adults living with co-occurring or complex needs conditions (i.e.: substance use disorders, developmental disabilities, and etc.)  CCS CARE services help SMI Adults experience the hope of recovery and live successfully in the community.   ​

  • Integrated Assessment, Evaluation, and Screening

  • Case Management

    • Symptom Management and Recovery Skills.

    • Direct Assistance with basic necessities of daily life.

    • Direct Assistance with performing daily living activities.

  • Crisis Intervention

  • Individual Therapy

  • Group Therapy

  • Parent/Guardian Therapy

  • Family Education/Support/Therapy

  • Psychiatric Services

These include but are not limited to:

  • Dialectical Behavioral Therapy

  • Acceptance and Commitment Therapy

  • Developmental Psychology

  • Persons Centered Therapy

  • Cognitive/Behavioral Therapy

Let's Work Together

We recognize that other therapeutic modalities may also be utilized to assure that persons receive the interventions that best fit their needs to help them achieve their recovery goals. The CARE team is made up of outpatient case managers and a clinical supervisor. The team is multi-disciplinary to best meet the needs of the persons served. CARE services are provided daily, always with someone available to handle emergencies.
